Man Develops Painful Rash After Eating Shiitake Mushrooms

Consuming undercooked or inadequately prepared food can lead to various health issues, and a recent case study published in The New England Journal of Medicine highlights the adverse effects of eating shiitake mushrooms. A 72-year-old man experienced a severe rash covering his entire back after consuming these common edible mushrooms, resulting in a trip to the emergency department.
Shiitake mushrooms, known for their earthy and meaty flavor, are frequently used in East Asian culinary traditions and are widely available. Within two days of eating a meal that included shiitake mushrooms (Lentinula edodes), the man developed an intensely itchy rash on his back, disrupting his sleep.
Upon visiting the hospital, doctors found that the patient had streaks of inflamed and swollen skin on his back and the upper part of his buttocks, resembling the marks of a whipping. After excluding other potential allergies, the medical team identified the condition as shiitake dermatitis. This dermatitis often presents with distinctive “whip-like” streaks on the body’s trunk.
Shiitake dermatitis was first reported in Japan in 1977 and is more commonly found in Asia, where shiitake mushrooms are a dietary staple. However, cases have been reported in other regions, including Europe, North America, and South America.
The rash associated with shiitake dermatitis resembles the skin conditions observed in patients undergoing chemotherapy with bleomycin, as well as those with dermatomyositis, an inflammatory disorder causing muscle weakness and skin rashes, and adult-onset Still’s disease, a form of inflammatory arthritis. This case serves as a reminder to ensure that mushrooms, like other foods, are adequately cooked to avoid potential health issues.
